Records Request

All patient care records are confidential and protected by privacy laws including HIPAA. Ridgewood Emergency Services has established a privacy and security policy to comply with these laws and to assure the integrity of your protected health information.

All requests for copies of records must be made in writing, signed by the patient or the patient's legal guardian and notarized. Only under a few exceptions, such as complying with a subpoena, can medical information be released without the consent of the patient.

All requests should include a contact phone number for verification. Please indicate how you would like the report sent to you (mail, or email.) Requests by attorneys should include a signed HIPAA or medical release form.

Records requests will be completed within 30 days of receiving all signed paperwork. A small fee may be charged in accordance with applicable NJ laws.
