How to Obtain A Certified (raised seal, original) Copy of a Vital Statistic
An application for a Vital Statistic record must be completed and sent along with proper ID and payment. The fee for each certified copy of a Marriage or Civil Union Certificate is $20.00 and the fee for a certified copy of a Birth Certificate is $25.00 for the first copy and $20.00 for each additional copy of the same birth certificate ordered at the same time. The fee for each certified copy of a Death Certificate is $15.00. Effective on November 30, 2010 a certified copy of a death certificate will be $25.00 for the first copy and $2.00 for each additional copy purchased at the same time. Payment by check or money order. Payment should be made to "Village of Ridgewood". Please enclose a self addressed, stamped envelope along with your application. Requests will be expedited as long as all pertinent information, proper ID and correct payment are enclosed.

Who may obtain a certified copy of a "Vital Statistic" ?
Subject of the Record (Birth, Marriage, Domestic Partnership) (see Note*)
Surviving Spouse (Death)
Child of subject
Grandchildren of the subject
Siblings (see Note*)
Persons requesting a Vital Statistic must provide the ID listed below. NO EXCEPTIONS.
Female applicants that are married and have assumed their husband’s surname, must also provide a certified copy of their marriage certificate when requesting a copy of their own birth certificate.
Individuals requesting a sibling's birth certificate must also provide their own birth certificate in addition to the other ID requirements.
Acceptable Forms of Identification
A valid government issued Photo ID (i.e. Driver's License) having Current Name and Address
A Photo ID and one alternate form of ID having Current Name and Address.
Two alternate forms of ID having Current Name and Address.
Alternate Forms of ID include:
Non-photo driver's license
Utility bill (within the previous 90 days)
Bank Statement (within previous 90 days)
Tax Return for current or previous year
NOTE* - ID must show a street address NOT a P.O.Box. If your driver's license is listed as a P.O.Box, two utility bills must be presented with a street address.
